The Providence Surgery Center charges a predetermined
fee for each type of surgical procedure. This fee includes operating
room and recovery room, nursing service, all supplies (excluding
implants), and medications. This does not include fees for your
surgeon, anesthesiologist, and pathology fees. These fees will be
billed separately, if applicable. Contact your insurance provider
in advance if you have any questions.
Please note:
~ Please contact your private insurance carrier if your policy coverage
states that pre-authorization or certification is necessary when
surgery is scheduled in a free-standing ambulatory surgery center.
~ Please bring a current insurance card with you to the surgery
center. The center will file the insurance claim for you. Most insurance
companies reimburse the center for surgical care less deductible
and coinsurance. The patient is responsible for the balance after
insurance payment or insurance denial.
~ Charges for cosmetic procedures and non-insured cases must be
paid in full on or before the day of surgery. Acceptable forms of
payment are: cash, personal check, cashier’s check, Visa,
or MasterCard. Prior financial arrangements can be made by calling

~ Balance is due within six months from the date
of service and monthly payments are expected.
