Fees & Insurance Information:
The Providence Surgery Center charges a predetermined fee for each type of surgical procedure. This fee includes operating room and recovery room, nursing service, all supplies (excluding implants), and medications. This does not include fees for your surgeon, anesthesiologist, and pathology fees. These fees will be billed separately, if applicable. Release of Information form:
The Release of Information Form is to be used when the patient would like to receive a copy of their medical records. This includes, but is not limited to; operative report, history and physical, anesthesia record, nursing documentation, and/or pathology reports.

Please fill out this form to the best of your ability, requesting specifically the information you need and where you would like the information mailed. It is important that you sign and date the bottom of your request, your signature is required and will be checked against your medical record signature.

NOTICE
We keep a record of the health care services we provide for you. You may ask us to see and copy that record. You may also ask us to correct that record. We will not disclose your record to others unless you direct us to do so or unless the law authorizes or compels us to do so. You may see your record or get more information about it at the Medical Records Department, Monday – Friday, 8 a.m. to 4 p.m.
